Daycare Agreement & Liability Release

Release of Liability

Outings are active and outdoors, and I take every dog's safety seriously. Like any outdoor play, they come with some normal risks. By signing, you understand and accept those risks, including:

• Off-leash hiking and play on private and public forested land
• A dog wandering off or not coming when called
• Scuffles or bites during play with other dogs
• Scrapes or other injuries from terrain, water, wildlife, plants, or weather
• Picking up an illness or parasites outdoors or from other dogs
• Riding in my vehicle to and from outings

You release High Bridge Dog Care and its owner, employees, and agents from all claims for injury, illness, loss, or death of your dog arising from my services, except those caused by gross negligence or willful misconduct.

You are responsible for any injury or damage your dog causes to people, other dogs, or property, and you agree to cover my costs if a claim is made against me because of your dog.

You confirm your dog has no history of aggression or biting toward people or dogs, except as noted on this form.

Health and Vaccines

All dogs must be current on rabies, DHPP, and bordetella vaccines. Because outings take place outdoors on trails and forest land, flea and tick prevention is strongly encouraged. High Bridge Dog Care is not responsible for fleas, ticks, or other parasites a dog may pick up on outings. You agree to let me know if your dog shows signs of illness, and sick dogs may not attend.

Veterinary Emergencies

If your dog is injured or becomes ill, I will try to reach you and your emergency contact first. If I can't reach anyone, I'm authorized to take your dog to your veterinarian or, if they're unavailable, the nearest appropriate clinic. You agree to pay for any veterinary care provided. Home Access

If I'm picking up or dropping off at your home, you authorize me to enter for that purpose and will text me any door or gate codes. I'll keep codes private and make sure your home is secured when I leave. I'm not responsible for theft or damage I didn't cause, or for anything your dog does in the home before pickup or after dropoff.

Outings and Care

• Outings happen rain or shine. I do my best to towel dogs off before drop-off, but they may still come home wet, muddy, and tired.
• I use treats during outings, especially for recall. Please list any food allergies in the form.
• I may refuse or end service at any time, including if a dog isn't a good fit for the group.

Payment and Cancellations

Payment is due after each outing by Venmo or Zelle, at my current rates. Please give me at least 24 hours' notice if your dog will miss a scheduled outing. Life happens, so you get 2 late cancellations per year at no charge. After that, late cancellations and no-shows are charged 50% of the outing rate.
